Are there any big differences between the Japanese and US versions of Forgotten Worlds?

I am not aware, but I guess the shop uses English instead of Japanese.

English text, but they couldn't be bothered to redo the spoken stuff. The PCE version also lets you pick between the original arcade voices (shop girl and player's grunts and screams) and new ones.

Too bad the shop girl doesn't grunt and scream too.

Yes, the PCE version was bundled with the Avenue Pad 3.

I believe the PCE version was also sold without it.

Yeah, it's just the shop lady's voice that's missing entirely. The rest of the stuff is there, using the audio from the arcade.
