Title: How/When was released System Card 1.00?

Hello there. Perhaps this is a stupid question but i´ve never seen a boxed one on ebay or yahoo japan.
I got mine inside the Altered Beast box in an ebay auction. I think it´s the only game that require´s this card. Am i right?
On Daisakusen says released as normal. Is this true? It seems a lot of information missed about this card.
Anyone has it boxed? Was released alone or with first cd units or ifus?

I think it actually was released with packaging and everything, but %99.9 of people got it packed in with the CDROM2.
