I've seen a lot of auctions or private sale did not include 3-button controller.  Did NEC release two versions, one with controller and one without?

If you mean bundled, dunno. If you mean two versions of the software, then no.

I'm sure it was sold unbundled, otherwise they'd be shooting themselves in the foot with a non-competitive price.

Actually, sunteam_paul is right, there's only one version of the software, but there was definitively a bundle - large box with an Avenue Pad 3 controller. I'm not 100% sure if the game was released on its own though, but I assume it did.
